Q.How should the attorney-in-fact sIgn when acting as my agent?

A.Any document being signed by an attorney-in-fact (on behalf of another) should not only include the attorney-in-fact's signature but also a notation that the document is being signed “as attorney-in-fact for” the Principal. If the attorney-in-fact only signs his or her own name, he or she may be held personally responsible for whatever was signed. We suggest using one of the following two forms:
